Working with local Volunteer agencies
Blacketts Medical Practice are proud of our links with local volunteer agencies.
We have a strong working relationship which involves us meeting regularly to share news about services and changes to pathways etc. This very useful information is then passed onto our GPs/Nurses and Admin teams at our regular meetings. Care Navigating is very important to us, we are happy to help and support patients and their families when care needs increase etc.
We offer Reception space for our colleagues from Darlington Association on Disability, Age UK, and Carers Support, Step Forward in Darlington and the Veterans Support Service, Ex-Forces Support to come in and speak directly to our patients. Feedback is very good and patients are very grateful for the information/support that they receive.
The Agencies are also help refer our patients onto social support groups e.g. walking groups, lunch clubs, knitting groups, Ukulele, Respite care, Befriender service, Computer and Internet group. Loneliness and isolation is very common with the elderly and we offer links from our website as well as leaflets in surgery offering help and support.
